If they're truly openminded, you might want to consider eComStation. You can use PostgreSQL as an SQL Server replacement, GPFRexx (yeah, I know, unfortunate name) as a VB replacement, and I suspect that with a little work you could find a good POS replacement as well... I'm 99% sure that IBM has one, though it would probably take some digging to find it. Here in Canada, you can find OS/2 all over the place (esp. in bars) as a POS system; all those touchscreens you see in them are all OS/2 systems.

One big advantage would be that the hospitals wouldn't see the value in keeping the workstations (or really delaying returning them) as they wouldn't see them as a cheap source of workstations as they would have a hard time recognising the base system.

OS/2 excels as an embedded system... it's worth consideration at any rate. Don't try to sell OS/2 though... sell eComStation; it doesn't have the negative brand associations that OS/2 has for huge chunks of the IT marketplace.
