Why is a Lieutenant General a higher rank than a Major General ?
whose mil. school career concluded with 3 possible appointments to West Point; declined.. I mean if they don't even know about lts. VS majors.

I'm guessing here, but it's probably two completely different meanings of the word 'Lieutenant', one being a rank, the other meaning assistant or deputy, as in Lord Lieutenant of a county, who is the Queen's representative when she does not attend an event, and her assistant when she does.
