PowerToys keyboard manager does the trick.

It is 40+MB and needs .NET, of course, but it's easy to do the mapping.
Alt (Right) + Up = PageUp
Alt (Right) + Down = PageDown
Alt (Right) + Left = Home
Alt (Right) + Right = End
Woot!
If PowerToys isn't loaded, the mapping reverts to the standard.
