I'm sure we can come up with a completely different top 25 list. Or two.

For my personally, the Iomega Jaz drive was much worse than the Zip drive (I had both). Until Iomega got greedy and cut too many manufacturing corners, the Zip drive was wonderfully reliable.

New Missing from the list
Early version of Visual Source Safe, the last couple versions of Visual Studio, Netscape Navigator 4.

And there are no programming languages or systems at all. HTX/IDC, C++, and client side VB script, the Windows API all come to mind readily as bad ideas and/or badly implemented. I'm sure I could go on in that direction for some time.
