New Other than Logitech most don't use the thumb . . .
. . except to hit the left button. Haven't had any problem there even the times I've had a hurt thumb (the stroke is just like hitting the space bar). I've used Kensington Orbit track balls for as long as I can remember.

New Space bar is the problem
My right thumb is used primarily for the space bar, and that's the motion that irritates it. I'm in the process of trying to teach my left hand to do the space bar, but it's very hard to change.
