The actual limits for Medicaid are rather complex and vary from state to state, but in general you have to have a monthly income in the $1000 dollar range. It is actually possible in some states to be working a mininum wage job full time and be taking home too much money to qualify for Medicaid.

New Lots of places do that sort of thing
At the other end of the spectrum from budget providers, there are various 'health resorts' in Europe that can do any number of cosmetic and other operations with very high quality, very low reporting and security that can keep even the paparazzi away. These places cater to media stars, politicans and the super rich, and prices reflect that.

Mexico has a lot of clinics that specalize in day visit surgery. Dental seems to be particularly hard hit, probably because the prices of dental work in the US have been inflated for so long. But there are lots of other places in Mexico you can go and get just about any procedure you want, no matter if you need it or the procedure does anything.

Other places specialize in particular types of work. Asia, particulary Bangkok, is the place to go for sex change operations. And there are hospitals in India and China that specialize in providing organ transplants to Americans, just don't ask too many questions about where the organ came from.
