I have one, it only gets used in emergencies, and if somebody needs to get in touch with me in a public place, they can talk to my voicemail.
You can talk on your cell phone in public as long as you give me my right to blow garlic-onion pastrami belches in your face all day long. It's about as annoying.

This is a little extreme
as I think it has the potential to put doctors out of touch.
I would prefer a device that automatically disabled ringers (enabling vibe mode at the same time).

I've not inadvertently left my phone on in the wrong place for 4 years (had it on in the cinema, it rang for ~2 secs before I killed the call).
The trick is to obtain a phone that does vibrate then ring with increasing volume.
99% of the time it'll never make a sound before you answer it or reject the call.
