I withdraw the comment of no reported deaths from flouride.

The comment I will add is that this spelled out as a serious case of neglect and goes to prove that people can die from blunders in administering public utilities and also from neglect by private companies trusted with the safe handling of dangerous chemicals (i.e. Bhopal, 10-mile Island).
