New Well, could be natural
There is considerable variation in ear shape.

That one's a bit unusual, but I've seen one - a good number of years ago, so surgery is extremely unlikely - where the ear was so thin it was translucent and the tubercle (which was unusualy open, almost flat) followed the leaf form so perfectly it had to be real. And the veins (this was a very translucent ear) followed as well.

People come in a lot of different shapes...

New Just remembered who it was
A Mayan that I used to know. Most Quiche Maya have nearly pointy ears. Handsome guy. Could have been the model for several ancient artworks, even though his forehead was only naturaly high and the actual model had had a board strapped to his head when he was an infant to get that look.

This guy helped me skip town for my first wedding. 25 miles over mountain, no roads or any other kind of non-foot access. Later told everybody I'm the fastest white guy he's ever seen. I don't feel too bad about it - the bride ditched too, in another direction.

So I'm absolutely certain it wasn't surgical - there are almost no medical facilities in the area, what little there is is dedicated to very basic essential care.
