New Re: User Interface Disasters 101
One software application that I worked with used "easy to remember" function keys to manage reports. The "Print Report" function key was next to the "Irrevocably Delete This Report and Get in Trouble with Your Boss" key.

Of course, this wasn't the worst problem. These same brainiacs made the customer phone number the primary key for the customer purchase history table.

New More death dealing keys:
I can't remember the name of the editor, but I was told this one in my UI class in college.

There is an editor wherein if you type the word 'exit', it means:

e - select entire document
x - delete entire selection
i - insert mode
t - 't'

Basically typing 'exit' replaced your entire document with the letter 't'...

New thought that was "edit"
..which makes it even funnier, because now the document you wanted to edit consists of a single letter "t".

Think this was mentioned in Bjarne 2.
