New Got reminded today of the solution.
AfterShokz.

They get plenty loud and sound surprisingly good. And don't block ambient sound.

They are, however, very sensitive to how they are positioned by your ears, so I don't know how practical they are in a car if there's lot of jostling, etc.

I haven't actually used mine much because of the hassle of trying to use multiple Bluetooth things at once. I haven't tried using them in the car.
