Report into a 1975 fire inside the Browns Ferry nucular station in Alabama. Yikes. Based on the summarized identical cavalier attitude the planet over, it is amazing there are not any more smoldering glow-in-the-dark holes around.

I like to think that people can learn from the past once they know about it. I would hope that the most effective training anyone could have would be a list of disasters and near disasters in the field with short explanations.

Lots of nuclear accidents have started because of "simple" stupid mistakes. We have been very lucky...

You use whatever tech you have available at the time. It is the removal of fire prevention measures, the near total lack of firefighting infrastructure, and the state of disrepair of whatever did exist, that beggars belief. (And the 90 minutes it took to call the fire deparment + the farce that followed...)
