New Just switched to ManageEngine ServiceDesk+
Installation was painless. Plenty of bells & whistles and works reasonably well so far. It does have a number of add-on modules (asset management, facilities management, ...) but we're not using those.
New I built one for a former employer
Designed the database, designed the screens and wrote the business logic for it. Not too hard.

Lots of places offer software for big bucks, little bucks, free in the cloud, etc. Just picked one from a Google search, for instance, http://www.polarsoftware.com/

Seems like you have quite a lot of choices.
