One of the latest Idiocies is a web-page application, hosted locally, that talks to a database over an SSH tunnel (yeah - brilliant). It doesn't know how to recover when the tunnel falls over - as it does from time to time because the other end is in a colo halfway around the world! So The Correct Solution is to re-implement this functionality on servers physically right next to the target database where there's already a login infrastructure for another closely related application. Funny: that's where I said it should have been in the first place! (fuckwits...)

Or the skunkworks reseller portal that entered production by stealth. Trouble is, now it's doing Naughty Things to the main database and has to be re-implemented - where it should have been in the first place.
