At a high level yes - details change often though. At a concrete level - no.
Do I have the materials and equipment I need to do my work right?
No.
At work, do I have the opportunity to do what I do best every day?
No.
In the last 7 days, have I received recognition or praise for doing good work?
No.
Does my supervisor, or someone at work, seem to care about me as a person?
Not particularly.
Is there someone at work who encourages my development?
Somewhat in so far is it involves learning more about the business.
At work, do my opinions seem to count?
Not if they diverge from "what we already know how to do".
Does the mission/purpose of my company make me feel my job is important?
Yes
Are my co-workers committed to doing quality work?
Yes
Do I have a best friend at work?
No
In the last six months, has someone at work talked to me about my progress?
Yes - was it accurate or was it fixing blame? - fixing blame - so perhaps NO.
This last year, have I had opportunities at work to learn and to grow?
Yes

Nope

Myself?

If I fight for it (I developed a reputation for shooting matches with my boss)

Dunno. We're financial company trying to switch from purcahsed software to home-grown. As such, we're bucking the industry's trend. Sometimes I think that the whole thing is just a set-up by some upper management, so that smemony somewhere can say "I told you so", fire the whole team and oust some VP who talks too much.
9. Are my co-workers committed to doing quality work?
Yes, when they recognize "quality". Putting "usleep(500)" in the code to avoid occasional crash on startup is considered too bad, rewriting the code is considered too dangerous, so we just have a shell wrapper to restart the process.
10. Do I have a best friend at work?
No
11. In the last six months, has someone at work talked to me about my progress?
Yes. But that was rather useless.
12. This last year, have I had opportunities at work to learn and to grow?
Hardly.
